A waiver will be granted if the following criteria can be met:

Attach proof of residing in Florida

Proof of residing in Florida must accompany this form. This may include but is not limited to a copy of Florida driver’s license, Florida identification card, or copy of lease, electric bill, or water bill (PDF Format Only).

Acceptable file types: .pdf. Maximum allowed file size: 2.50MB

Attach proof of 100% service-connected disability

The individual can provide proof they have a 100% service-connected total permanent disability rating for compensation. A valid identification card by the Department of Veteran's Affairs can be used as proof and must accompany this form (PDF Format Only).

Attach proof of eligibility

The individual can provide proof of eligibility for the Post 9/11 GI Bill which is a certificate of eligibility and must accompany this form (PDF Format Only).
